  • Appointments

    • Schedule by calling listed physician number and speak with the receptionist; Please have your insurance card handy when calling so information can be collected prior to visit.
    • Walk-ins may be accepted if you are willing to wait, but it’s always a good idea to call ahead.
    • To cancel an appointment, it is requested you call 24 hours in advance.

    New Patients

    • Please bring your insurance card with you to every visit. If your insurance has a co-pay, please be prepared to pay at the time of service.
    • Some physicians require you to bring in prescription bottles of any medications you are taking.
    • New Patient Information Forms are required to be filled out and signed. These forms may be downloaded from this site. Please click on the provider you will be seeing and then click the forms on the top of the provider profile (located by their name).

    Medical records

    To obtain medical records:

    • Sign a medical records release form and send to physician’s office
    • Indicate physician who is to receive record
    • In some cases there may be a charge.

    Prescription Policy

    There is a 24 hour period for a call in prescription request.

    Special Services (Radiology, EKG, Lab, Women’s Services)

    The convenience of an office complex that is physically connected to the hospital is extremely advantageous, as well as the availability of the diagnostic services right here in our building.

    • Radiology and laboratory services are located on the second floor.
    • The Connection for Women (located in Suite 301) provides diagnostic ultrasound, mammography, bone densitomety screening, and massage therapy.  
  • Office Hours

    Regular hours are 8 a.m. to 5 p.m.
    (Individual physician office hours may vary)

    The St. Lawrence Health Alliance offices are closed on the following holidays:

    • Memorial Day
    • Fourth of July
    • Labor Day
    • Thanksgiving
    • Christmas Day*
    • New Year’s Day*

    *Our offices will close either Friday or Monday if these holidays fall on a weekend.

    Emergency/After Hours Care

    For emergencies, please dial 911 or go to your local emergency room.

    Claxton-Hepburn Convenient Care, located at the Ogdensburg Family Practice Building, 921 State Street, across from Rite Aid in Ogdensburg is staffed with nurse practitioners and physician assistants, with Dr. Christopher Comeau serving as the Medical Director.

    Claxton-Hepburn Convenient Care provides non-emergency care to persons of all ages, Monday through Friday from 5 p.m. to 9 p.m. and on Saturday and Sundays from 8 a.m. to 4:30 p.m. Click here for more information.

    Appointments may be made by calling 393-1715. Most insurances accepted.

    We offer treatment for urgent care conditions, including:
    • Colds
    • Earache
    • Flu
    • Mild (non-emergent) allergic reactions
    • Minor cuts and scratches
    • Rashes
    • Sore throat
    • Sprains and strains
    • Urinary tract infections

    Our clinicians are adept at diagnosing problems quickly and providing the right treatment plan to help you feel better. Your doctor will be informed of any treatments you have in our clinic, so you can go to your doctor for any follow-up care you might need.

    And remember: If you are dealing with a life-threatening emergency, call 911 or go to the emergency room right away.

    The staff in our clinic can also help you with:

    • Blood draws
    • EKGs
    • Laboratory testing
    • Physicals, including DOT physicals

    If you need testing, bring a note from your doctor outlining the help you need, and we’ll take care of the issue right away. Any test results will be shared with your doctor, so you can follow up at your convenience.

  • General Billing Information

    • We accept MasterCard, Visa, Discover and American Express.
    • Most insurances accepted
    • Self-pay due at time of visit unless other arrangements made. A 15% discount will be offered to a self-pay patient that is able to pay the entire balance on the date of service.
    • Unpaid balance due within 30 days
    • Fee information/special payment plans upon request
    • All insurance will be billed as a courtesy to the patient

    In compliance with State Law, St. Lawrence Health Alliance and Claxton Medical PC will provide a list of prices to each patient or community patient upon request.

    Understanding Prices

    While healthcare charges can be confusing in a consistently changing environment, the team at the St. Lawrence Health Alliance and Claxton Medical PC is committed to keeping our prices among the lowest in our community. We are also committed to correct and transparent billing. If a service appears on a bill that was not provided, call 315-713-6611 and our team will investigate immediately. We charge patients and insurance companies the same price for individual services. In addition to office visit charges, patients may be billed by other professionals who participated in providing care.

    St. Lawrence Health Alliance and Claxton Medical PC Deliver Transparency in Pricing

    In compliance with state law, St. Lawrence Health Alliance and Claxton Medical PC provide a price list available to each patient and community member upon request. This list contains our estimated charges for services including:

    • Physician Office Visits
    • Claxton-Hepburn Convenient Care

    The charges are the same for all patients but a patient’s responsibility may vary, depending on payment plans negotiated with individual health insurers.

    These prices are an estimate based on the historical data as of December 31, 2014. For specific estimates, please contact our billing coordinator’s office at 315-713-6611 to be provided an individual cost estimate.

Charges can fluctuate based upon the care needed, the personnel resources, the intensity of care and the amount of time needed to provide treatment. The following charges do not include fees for drugs, supplies, or additional ancillary procedures that may be required.

    Billing Policy Summary

    If you have questions or need assistance with your bill, our Billing Team is available to assist you. The Medical Biller can furnish information, make payment arrangements and help you resolve insurance billing problems.
